FUNERAL DIRECTOR REFERRAL
Funeral directors should respect the family’s wishes
to donate, and use their relationship with the donor
family to facilitate the recovery process. Ideally,
funeral directors will view donation as an integral
part of the donor family’s efforts to deal with
the loss and as an aid in the progression of the
grieving process.
Please Call: 800.366.6777
(Prior to embalming.)
Information needed:
1. Name and age of decedent.
2. Date/time of death.
3. Cause of death.
4. Known medical/social history.
5. Family contact information.
A trained CORE specialist will screen the donor
for medical suitability, obtain a thorough medical/
social history, record family authorization, and
update the funeral director.
Organ procurement organizations (OPOs) and
funeral directors have worked together for years
to promote and enhance the gifts of organ,
tissue and cornea donation.
We support ongoing efforts to provide the gift
of life in the wake of tragedy.
Comprised of practicing morticians, the CORE
Funeral Directors Task Force advocates that
funeral professionals adopt an ethical code
of conduct designed to facilitate and promote
cooperation with ongoing organ, tissue and
cornea procurement programs.
As a trusted funeral professional, we ask that
you support and endorse organ, tissue and cornea
donation. We understand that working with
those who have lost a loved one provides you
with a unique opportunity to make a difference.
You have experience working with families and
friends as an advisor, confidant or friend during
a complex time when important, timely choices
are required for burial and funeral arrangements.
If the departed is not a designated donor,
the family may request your advice.
To facilitate your efforts in counseling families
about the steps involved in preparing the body
following an organ, tissue and/or cornea recovery,
the CORE Funeral Director Task Force is prepared.
